Confirmed, it will take weapon from green to yellow 100% of the time.

You can NOT Upgrade a Ultimate item with this stone, it will consume the Item and nothing will happen, the System will say that it was successful but nothing did change and Millions are gone.

You can't because it is already Ultimate, what should happen when you use it on an ultimate item?
